Department of Patient Information

There are three main sections under Patient Information Department:

• MEDICAL REPORT SECTION
• RECORD MANAGEMENT SECTION
• STATISTIC SECTION

MEDICAL  REPORT SECTION

Medical Report Section service is to provide medical report application, specialist medical report, post mortem report, insurance claim and also medical leave certificate clarification. This section also has responsible in organizing Medical Board Meeting and PERKESO Medical Board meeting.

Guidelines in Applying Medical Report

• Applicant/ patient must fill ‘Borang Keizinan Permohonan Laporan Perubatan (BKMIS-E034-E06)
• Please submit together with:
  1. Copy of Identity card
  2. Copy of UMMC Treatment Card
  3. Copy of Medical Leave Certification (if needed)
• For patient under 18 age, Next of Kin/ Guardian must completed  Surat Permohonan Laporan/Surat Perakuan Perubatan (BK-MIS-034)

Guidelines in Applying Post Mortem Report

• Applicant must fill in ‘Borang Keizinan Permohonan Laporan Perubatan (BKMIS-E034-E05)
• Please submit together with:
  1. Copy of Identity card
  2. Copy of UMMC Treatment Card (if any)
  3. Copy of next of kin identification card
  4. Foreigner applicants: To submit consent letter from Embassy.

Guidelines for Insurance Claim Purpose
(Ward admission, Accident. Labour Form and PKS6 Form)

• Applicant must fill in ‘Borang Keizinan Permohonan Laporan Perubatan (BKMIS-E034-E05)
• Please submit together with:
  1. Copy of Identity card
  2. Copy of UMMC Treatment Card (if any)
  3. Copy of Medical Leave Certification (if needed)

Guidelines for Confirmation Medical Leave Certificate/ Sick Acknowledgement To Attend in Court

• Applicant must fill ‘Permohonan Pengesahan Sijil Cuti Sakit’ form (BK-MIS-036-E01)
• Please submit together with:
  1. Copy of Identity card
  2. Copy of UMMC Treatment Card (if any)
  3. Original Copy or Copy of Medical Leave Certification for confirmation

Guidelines in Applying Specialist Medical Report

• Applicant must fill in ‘Borang Keizinan Permohonan Laporan Perubatan (BKMIS-E034-E05)
• Patient required to attend an examination on given date.
• Fee of RM 200 is required for advance payment.  Upon completion of Specialist Report, the remaining charges will be informed.

RECORD MANAGEMENT SECTION

This section manage all the record of patient information (physical and electronic) and handling all case related to EMR system. In managing the patient information, department has applied the PDPA policy for patient information protection.

STATISTIC SECTION

The function of Statistic Section is to provide  statistic of inpatient, outpatient, death, birth and also infection notification to Ministry concern. In line with International Statistical Classification of Diseases and Related Health Problems (ICD-10- Revision 10th), our staff to code accurate and quality disease diagnosis.
