Department of Nursing



Since the year 1966, the Nursing Administration has gone through a lot of changes in its organisation. In the beginning The Nursing Administration was lead by The Nursing Director (till 1969 name changed to Chief Matron) and assisted by 3 Grade I Nursing Supervisors, who manage the Phase 1, 2 and 3 services.

This covers every Nursing Unit in the Phase. The First phase contains Surgical Unit, Medical, Orthopaedic, ENT and Eye Ward.

The second phase contains Obstetric and Paediatric and third phase contains all the polyclinics and Accident and Emergency Unit. Every First Grade Nursing supervisor is helped by 2 Grade II supervisors. They are all known as Matron. All responsibilities are given according to the unit specifications. If there arises problems which cannot be solved at Grade I Nursing supervisor or grade II supervisors, it will then be referred to the Chief Matron.

In 1997 the Hospital started to follow corporate system, so the administration of nursing was change to different units of responsibilities. The Nursing administration was divided to four functions and headed by 4 Nursing Officers (formerly known as Matron) with the addition of 1 Nursing Officer for Trauma and Emergency services on the 1 August 2006.

As from 2001, an extension of nursing function where new post is established - “Clinical Nurse Specialist” (Pakar Jururawat Klinikal) working with the Ward/Unit Managers (formerly known as Sister) in the management and care of patient especially with specialization.

Generally the Nursing Administration are divided into the following activities/responsibilities:

  1. Nursing Policy
  2. Inpatient Services 
  3. Critical and Operation Theatre Services
  4. Ambulatory Services
  5. Trauma and Emergency Services


1. NURSING POLICY

  • Responsible for the formulating, reviewing nursing policies.
  • Participate in reviewing and formulating organizational policies.
  • Plan manpower requirement for nursing administration.
  • Plan and recommend placement of new staff nurses to all nursing unit
  • Identify needs for the staff development program
  • Plan orientation program (nursing group)
  • Plan and facilitate nursing research and recommendations to improve the standard of nursing service.
  • Coordinate and organize in the preparation of the facilities for the MBBS and Masters exam.
  • Be involved with the Quality Committee, Training Committee, PPUM Policy Committee, OSHA, Medical Enquiry etc.


2. INPATIENT SERVICES

  • Responsible for the coordination and management of patient care for all in-patients except the Critical services such as ICU, CCU, CICU, Paediatric 1, SCN and Labour Ward.


3. CRITICAL AND OPERATION THEATRE SERVICES

  • Responsible for the coordination and management of patient care for all Critical services such as ICU, CCU, CICU, Paediatric 1, SCN, Labour ward on operation theatre.


4. AMBULATORY SERVICES

  • Responsible for the coordination and management of outpatient services inclusive of Day Care.


5. TRAUMA AND EMERGENCY SERVICES


GENERAL WARD ADMISSION INFORMATION

  • All patients are admitted to the wards through the various clinics, the Primary Care Unit, the Day Care Centre and the Trauma & Emergency Unit. Direct Admission to the ward is allowed only with prior arrangement with the specialist and ward concerned. Generally patients are admitted by rotation to the general wards.

 

SERVING OF MEALS: 
Breakfast - 7.30 am to 8 am
Lunch - 11.30 to 12.30 noon
Afternoon tea - 3 pm to 3.30 am
Dinner - 6 pm to 7 pm

 

GENERAL WARD REGULATIONS:

  1. All patients are to wear hospital garments and a name tag during their stay.
  2. They are to bring the following:-
    1. Their own toiletries such as tooth brush, tooth paste, comb, powder, soap, towel, slippers etc.
    2. Medications that they are currently taking.
    3. Guarantee letter (if any) or other relevant documents (e.g. Pension card or social welfare card)
  3. Permission from the nurse is required whenever patient wants to leave the ward.
  4. Patients are advised not to bring in valuable items such as jewellery and excess cash.
  5. Handphones are allowed to be used at own risk and responsibility. Electrical charging of handphones are not encouraged. (? To allow or not to clear with management)
  6. No smoking is allowed.
  7. Taking photograph is not permitted, except with permission from the Hospital Authority.
  8. A relative/career is allowed to accompany the patient who is ill/disabled or with special needs with permission or discretion of the ward manager.
  9. Parents or a lodger is encouraged in the Paediatric Wards.
  10. No visitors are allowed during doctors’ ward round.
  11. Written permission is required to bring in home cooked food. Only halal food is allowed.

*Notes:- Specific regulation in certain wards (e.g. Psychiatry and Paediatric) are to be followed.

ORGANISATION CHART

The ward manager together with the ward consultant is responsible for the smooth running of the ward. There are two ward managers per floor for wards of 56 beds. The wards with 28 beds or less will have only one ward manager. The ward manager works either office hours or on administrative duties which may be afternoon or night duty. Reporting to them are the staff nurses, assistant nurses, clerks and ward assistants.

 

HIGHLIGHTS OF WARD ACTIVITIES MEDICAL UNITS

WARD 13UA (Geriatrics)

  1. The 30 bedded wards specialized in caring for the geriatric patients which was started in 1st March 1999.
  2. All high dependent nursing areas are equipped with close circuit TV monitoring so that patients can be observed from the nurse counter.
  3. A rehabilitation unit is attached to the geriatric ward and provides wide range of physiotherapy and occupational therapy facilities.
  4. The ward and toilet facilities are specially designed to assist the frail elderly patients in their daily living activities.
  5. The nursing care provided are assessing and planning needs for the elderly patients and giving care of assisting patient in carrying out activities of daily living (ADL).
  6. Care education and teaching are carried out to prepare families to care from patient on discharge.


WARD 13UB (STROKE & REHABILITATION)

  1. There are 3 single, 3 double, two of 4 bedded rooms installed with negative pressure for caring for the infectious disease patients (i.e. tuberculosis). Doors to these rooms are maintained closed at all times to achieve the recommended negative pressure.
  2. Sleep Apnoea test is conducted in the ward as an investigation for Obstructive Sleep Apnoea patients.
  3. Patients are admitted for investigation, procedures, medical care and also treatment for specific and general medical problems.
  4. A respiratory day care service is also available in the ward for patients undergoing chemotherapy.


WARD 11U (INFECTIOUS DISEASES, CARDIAC, ONCOLOGY)

  1. The 28 bedded ward is specially designated to care for dengue patients and also infectious diseases patients.
  2. Besides infectious diseases cardiac and oncology patients are cared in this ward.
  3. Patients admitted are for investigation, procedures, medical intervention and nursing care.


WARD 8TD (NEPHROLOGY, ENDOCRINE, RHEUMATOLOGY)

  1. The ward provides services for Endocrine, Rheumatology and General Nephrology care.
  2. Post operative renal transplant patients are nursed in this ward till discharge.
  3. Ventilated patients requiring haemodialysis will be performed in this ward.
  4. Patients admitted are for investigation, procedures, medical intervention and nursing care.


WARD 8TE (DIALYSIS CENTRE)

  1. This is a dialysis day care center which provides haemodialysis, continuous ambulatory peritoneal Dialysis (CAPD), Automated Peritoneal Dialysis (APD) and continuous venovenous haemodialysis, (CVVHD) for critically ill patients in the Intensive Care Unit.
  2. Pre and post renal transplantation management are also available.
  3. Follow up services include haemodialysis clinic, CAPD clinic, transplant clinic and vascular access clinic.


WARD 6TE (CLINICAL ONCOLOGY)

  1. This ward (4 beds) provides brachytherapy treatment to patient with cancer of cervix and radioiodine therapy for patients with cancer of thyroid.


WARD 6TD (HAEMATOLOGY, ADULT BONE MARROW TRANSPLANT, HAEMATO-ONCOLOGY)

  1. It has a ward and a day care unit for patients with hematological and haemato-oncology diseases.
  2. The first adult allogenic bone marrow transplantation in Malaysia was carried out in November 1993.
  3. The first autologous peripheral blood stem cell transplantation in July 1997.
  4. The first non myeloablative peripheral blood stem transplantation in year 2001.
  5. Till now, a total of 97 bone marrow transplantations and peripheral blood stem cell transplantations have been done with comparable results as reputable overseas transplant centres.
  6. The day care centre operates daily (except Friday, Sunday and public holidays) between 8 am to 6 pm. It caters mainly for patients who require blood transfusion, investigation and chemotherapy.


SURGICAL UNITS: WARD 10U (GYNAECOLOGY & GYNAE-ONCOLOGY)

  1. This ward provides services to general gynaecology and gynae oncology patients.
  2. A Day Care center is available to provide day care gynae oncology services.


WARD 9U (GENERAL ORTHOPEDIC)

  1. This 56 bedded ward admits general orthopedic cases and oncology care.
  2. Pre operative and post operative care are provided in this ward.
  3. Patient are taught how to self care, rehabilitated and mobilized with orthopedic appliances before discharge.


WARD 8U (HAND & MICROSURGERY, SPINAL REHABILITATION, GENERAL ORTHOPEDIC)

  1. This 56 bedded ward provide management of general and specialized orthopaedic cases.
  2. A total of 12 beds are allotted for spinal and spinal rehabilitation cases.
  3. Pre operative and post operative care are provided in this ward.
  4. Patient are taught how to self care, rehabilitated and mobilized with orthopedic appliances before discharge.


WARD 7U (GENERAL SURGERY, PALLIATIVE CARE)

  1. The 56 bedded ward provides high dependent care and general pre & post operative care for surgical patients.
  2. Special endoscopic and radiological procedures pre and post care are also provided in this ward.
  3. Chemotherapy and follow up care are conducted in this ward.


WARD 6U (UROLOGY, NEUROSURGERY, GENERAL SURGERY – VASCULAR, CARDIOTHORACIC, PLASTIC & BURNS

  1. This 56 bedded multi disciplinary unit caters for specialized surgical cases.
  2. Pre operative and post operative care are provided in this ward.
  3. Day care services such as Urodynamic, Trans Rectal Ultrasound Biopsy, Extracorporeal Shock Wave Lithothripsy and Deep Brain Stimulation are provided too.


WARD 5U (NEUROSCIENCE MEDICAL, NEURO SURGICAL WARD)

  1. The neuron surgical and medical services is equipped with intensive care and high dependency facilities to care for the critically ill neurological patients.
  2. Special procedures such as Intracranial Pressure Monitoring (ICP), Continous Electroencephlogram (ECG), and Plasma Exchange are performed in the wards.
  3. The care givers are supported and taught in coping with care of neurological impaired patient for long termed care and rehabilitation.


WARD 7TE (OPTHALMOLOGY)

  1. This 28 bedded ward provides care for patients with elective and emergency problems of the eye.
  2. Preoperative and post operative care are also provided. WARD 7TD


(OTORHINOLARYGOLOGY, DENTAL, ONCOLOGY)

  1. This 28 bedded ward provides care for the ear, nose, throat and dental cases.
  2. Chemotherapy and palliative care are also provided.


WARD 3TE (PSHCYIATRIC)

  1. This ward is divided into two areas with 20 beds on each side to nurse male and female patients who requires psychiatric care.
  2. Electro Convulsive Therapy are conducted three times per week or as required by patients.
  3. Regular Psycho education program conducted in ward to help patient to cope with illness and comply to medications.
  4. Note : Ward Maternity 2 is temporary closed for renovation purpose.


PAEDIATRIC UNITS: WARD PAED 6 (ONCOLOGY, OPHTHALMOLOGY, DENTAL, OTORHINOLARYGOLOGY, PAEDIATRIC DAY CARE)

  1. This ward provides services pediatric oncology services, pediatric ophthalmology, otorhinolaryngology and dental.
  2. The day care services provides care for thalassemia children and procedures such as blood investigations, transfusion, bone marrow examination, chemotherapy, care of indwelling central venous catheter and tissue typing procedures (HLA typing and DR typing) are also carried out.


PAEDIATRIC WARD 5 (GENERAL MEDICAL, ORTHOPEDIC, ENDOCRINE, SCAN CASES)

  1. This ward provides care to the older group of children and adolescent.
  2. Besides caring for the SCAN children the ward also provides care for general medical, orthopedic and endocrine cases.


PAEDIATRIC WARD 4(GASTROLOGY, RESPIRATORY, INFECTIOUS DISEASE)

  1. This 20 bedded ward provides care for children with gastro-hepato, and chronic respiratory problems.
  2. All children with infectious and diarrhoe diseases are cared in isolation rooms in this ward.


PAEDIATRIC WARD 3 (GENERAL SURGERY, PLASTIC, NEUROSURGERY, CARDIATHORACIC)

  1. This 20 bedded ward provides care to neonates and the babies with cardiac and respiratory illnesses.
  2. Preoperative and post operative cases of eye, ear, nose, throat and dental are cared in this ward too.
  3. High dependency care are provided for those post Intensive Care children of the same age group who are suction, oxygen and ventilator dependent.
  4. Health education and care education are carried with mother to prepare them for home ventilation care.
