Department of Patient Information



History

The Patient Information Department (previously known as Medical Record Unit) was set up in 1966 and was located at the Faculty of Medicine, University of Malaya. The department was moved to the Ground Floor of the Tower Block when the hospital was built in 1967. Since February 2004 the department has been moved to the first floor, Trauma and Emergency Building. Since it was established the department has been lead by 4 Head Department.

iii) Datin Josephine Sinnathuray (1994 – 1999)
iv) Mr. Teh Ah Huat (1999 until now)

Introduction

The Patient Information Department main objectives are to provide comprehensive, accurate and up to date information for purpose of treatment services to inpatients and outpatients and for purpose of research to be carried out by the academic departments of University Malaya Medical Centre.

The departmen are also need to manage centralized medical record system ensuring confidentiality, safe, custody, quick retrieval and proper control of movement of all medical records in the hospital.

Patient Information Department is headed by Mr Teh Ah Huat, System Analyst Officer F52 and he is helped by Encik Amran bin Said, Administrative Officer N44 who is also responsible for Record Management Section.

Record Management Section

Department main services are to supply patient folders for out- patient and in-patient. Department has to ensure that 97% of the requested folders achieved.

Department is open from 8.00 am until 9.00 pm everyday except on weekend and public holiday.

Statistic Management Section

Function of Statistic Management Section is to provide accurate statistic of inpatient, outpatient, death, birth and also notification of infection to Ministry concern. Statistic Management also handle request from internal and external for research.

  1. Provide accurate and informative statistic.
  2. To code accurate and quality disease diagnosis within 5 working days.
  3. Provide assistance in research with responsibility and concern.
  4. Nourish cooperation element within staf.

Medical Report Section

Function of Medical Report Section is to provide service to public for applying medical report, specialist medical report, post mortem report, insurance claim and also clarification of medical leave certificate. Below are the guidelines when applying for the above matter and also charges when making application.

Guidelines Applying For Medical Report

Things that are needed to be ready when applying at Medical Report Counter

  • Applicant/patient must fill “Surat Permohonan Laporan/Surat Perakuan Perubatan” form (BK-MIS-034)*
  • Applicant is require to submit treatment information with complete document including treatment card, date and place patient receive treatment.
  • Payment for application for report must be settled upon requesting.
  • Copy of medical leave certificate (if needed) * “Surat Permohonan Laporan/Surat Perakuan Perubatan” (BK-MIS-034) must be completed by patient (for patient age 18 above).

For patient age 18 below “Surat Permohonan Laporan/Surat perakuan” (BK-MIS-034) must be completed by next of kin/guardian.

BK-MIS-034 form could be obtain at Medical Report Section counter.

If all the documents above are not provided, your application will be declined.

Guidelines For Post Mortem Report

  • Applicant must fill “Surat Permohonan Laporan/Surat Perakuan Perubatan” form (BK-MIS-034)*
  • Application for foreigner should have consent letter from the Embassy.
  • Applicant is require to submit treatment information with complete document including treatment card, date and place patient receive treatment.
  • Applicant need to submit copy of next of kin identification card
  • A copy of death certificate.
  • Payment for application must be settled upon requesting

Guidelines Applying For Insurance Claim
(Ward admission, accident, labour form and PKS 6 form)

  • Applicant/patient must fill “Surat Permohonan Laporan/Surat Perakuan Perubatan” form (BK-MIS-034)*
  • Applicant is require to submit treatment information with complete document including treatment card, date and place patient receive treatment.
  • Applicant need to submit copy of patient’s identification card or next of kin.
  • Applicant need to submit a copy of medical leave certificate.
  • Payment for application must be settled upon requesting.

Guidelines For Confirmation Medical Leave Certificate/Sick Acknowledgement To Appear in Court

  • Applicant must fill “Permohonan Pengesahan Sijil Cuti Sakit” form (BK-MIS-036-E01)
  • Please bring the original medical leave certificate or it’s copy for confirmation.
  • Applicant is require to submit treatment information with complete document including treatment kad, date and place patient receive treatment.
  • Fees will be charged according to the amount of medical leave certificate.

Guidelines Applying For Specialist Medical Report

  • Applicant apply for specialist medical report
  • Applicant/patient must fill “Surat Permohonan Laporan/Surat Perakuan Perubatan” form (BK-MIS-034)*
  • Fee of RM200 is require for advance payment.
  • Patient is required to attend for examination on a fixed date.
  • Upon completion of Specialist Report, the remaining charges will be informed.

Steno Pool Section

Steno Pool Section Steno Pool Section at Patient Information Department provide typing service for medical report (request from lawyers, insurance company, police and individual) and case summary. Service provided is from 8.00 am till 5.00 pm. Request from lawyers, insurance company, police and individual will be processed by Medical Report Section and after the medical reports have been written by doctors, it will be sent to Steno Pool to be typed before sending out.
